Interfacility from Maple Grove SNF to ICU. 72-year-old woman, 2 days of fever and cough, now hypotensive. Sending RN: 'We gave 500 mL NS. BP is still 78/40. No cultures. No antibiotics.'

Hospitalist: suspected sepsis, no central line, 22g in the hand. Allergies: penicillin rash as a child.

She is on a facility mattress, delirious, bounding then thready pulse, hot skin, productive cough. Family wants her 'to go to the good hospital.'
