48-year-old woman, day 2 after hysterectomy, sudden syncope and hypoxia on the ward. Sending ICU wants ECMO center. She is on 40 L HFNC.

CT pending — too unstable. Echo: dilated RV, septal bowing. BP 70s on phenylephrine.

Anxious, grey, clear lungs, loud P2 if you listen. Abdominal dressing clean and dry.

ARRIVAL HR 128, BP 74/52, RR 36, SpO2 84% HFNC, ETCO2 22, GCS 14.
