34-year-old woman, known severe asthma, three nebs at home, now tripod and tiring. ALS on scene with continuous albuterol. They called you because she is getting quiet.

She can only say one word. Accessory muscles. Quiet chest. Sweating. Looks at you like she knows.

ARRIVAL HR 132, BP 158/90, RR 34 then slowing, SpO2 86% neb, ETCO2 58 with shark-fin.
